Patient Registration Form

All established patients who have not been at the Baton Rouge Clinic since August 7, 2012, and any new patients are encouraged to download, complete and bring this patient registration form the next time they come to the clinic for either Lab, X-ray or Physician services. This Patient Registration form is to be completed only one time for each patient. The form is available at the link located below.

It is also advisable to either bring along all of your medications or a complete list of all medications, and the dosage and frequency that you are currently taking (regardless of which physician prescribed the medication for you).
